Volunteer Forms

• Applications accepted throughout the year
• Must commit to 4 hours a week
• Over 18; Not currently enrolled in college or graduate school
• Required: State ID (permit or passport works)
• Must complete health screen

Masters and Doctoral Internships: Unpaid positions available.

• Accepting Freshman and Sophomores only, who can commit to at least 3 consecutive years of volunteering (Fall and Spring semester)
• Minimum of 50 hours a semester
• Currently enrolled in college or graduate school
• Required: State ID (permit or passport works)
• Must complete health screen 

Year-Round High School Volunteer Program: https://www.volgistics.com/appform/1087073497

• Open to incoming 9th-12th graders
• 3.0 GPA, Letter of Recommendation from one teacher
• Students are required to volunteer year-round
  • 2 hours/week (Fall & Spring), 4 hours/week (Summer)
    • Total - 30 hours (Fall & Spring), 60 hours (Summer), or 120 hours/year

Exposure to different healthcare careers with hands on experience  

• Open to incoming 9th-12th graders
• 3.0 GPA, 400-word essay, Letter of Recommendation from one teacher
• Monday-Friday, 8am - 12pm
