Where does my health information on MyDCH come from?

All of the information in MyDCH comes from your Electronic Health Record created during your stay at a DCH Health System facility. You will have access to the most accurate, up-to-date information possible.

How do I log into the MyDCH portal?

Simply enter your user name and password. Click here to log in.

Can my family access my portal?

Yes, you can give family members, such as parents or health care proxies, access to the information on your MyDCH portal. This needs to be done in person at a DCH facility, and it requires your written consent. Click here to download the Patient Portal Proxy Access Form

Is my information safe?

Yes. MyDCH passwords are encrypted, and URLs are rewritten so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your MyDCH portal. Also, a timeout feature protects your information if you leave your portal page open.
