Northport Medical Center Medical Records Office Closing

As of March 1, 2024, the Medical Records Office at Northport Medical Center is closing.

Here’s how to get your medical records after March 1.

Online (quickest and easiest way)

• Visit the MyDCH Patient Portal to view your information from recent DCH visits. Scan the QR code at right. [insert QR code https://www.dchsystem.com/patient-portal/mydch-hospitals/]
• Want a more comprehensive record over time? Use the Download Medical Record button on the Patient Portal. Once you log in, locate the button under the Health Record section (right side of page).
• You may also request your records through the online record request tool. Scan the QR code at right. [insert QR code https://www.swellbox.com/dchsystem-wizard.html]

In Person

Visit Medical Records (HIM Department) at DCH Regional Medical Center. The office is on the northeast corner of the campus in the Medical Office Building (University Boulevard side of campus).
