Qualifications:
- Pharm.D. from an ACPE-accredited school of pharmacy or a B.S. in Pharmacy from an ACPE-accredited school of pharmacy with equivalent clinical experience
- Completion of an ASHP-accredited or candidate status PGY1 pharmacy residency program
- Licensure to practice pharmacy in the state of Alabama prior to or within 30 days of program start
- Submission a letter of intent describing the candidate’s interest in critical care, research interests, career goals and interest in DCH for PGY2 Critical Care Residency Training
- Submission, via Phorcas, of a curriculum vitae and recommendations from three preceptors or supervisors. At least two of the references should be from a preceptor or supervisor who can directly comment on clinical and practice skills.
