Community Health Needs Assessment

Every three years the DCH Health System conducts an assessment of the health and wellness needs of community members as well as the available resources to fulfill those needs. From that effort, the Community Health Needs Assessment is developed. This document acts as a guide to develop or support programs and services to help fulfill the identified needs.
